Why your pigmentation keeps coming back

A patient arrives frustrated. She trains four times a week, eats carefully, and her legs still feel heavy and look puffy — worse in the evening, much worse after a flight. She has come in to ask about body contouring.
We measure first, and about a third of the time the measurement changes the plan entirely.
Fat and fluid look similar and feel similar to the person carrying them. They respond to completely different treatment. Buying a course of contouring for what is actually lymphatic congestion means spending several months and a significant amount of money on the wrong problem — and concluding that contouring does not work.
The measurement. Segmental bioelectrical impedance separates muscle, fat and water and reports them limb by limb, in under a minute. It is definitive and it is the first thing we do.
The clinical signs. Fluid retention is typically worse in the evening than the morning, worse after travel or a salty meal, and often leaves a brief indentation when you press it. Fat does none of those things — it is the same at 8am as at 8pm.
Lymphatic drainage moves it toward the nodes where it is cleared. It is the one treatment in this clinic where patients routinely notice the difference before they leave the room. A course of six over three weeks, then monthly maintenance if your travel and desk hours warrant it.
The non-clinical half matters just as much: hydration — counter-intuitively, drinking more reduces retention — calf activation through the day, and reducing sodium. Most of the maintenance is done by you rather than by us.
Then contouring is a reasonable option, provided you are already reasonably close to your target weight and the issue is a localised pocket rather than overall composition. It is not weight loss, and it will not produce a change on the scale. What it produces is a change in how clothes fit, of roughly twenty to twenty-five per cent of the fat layer in the treated area.
The most common finding is a genuine localised fat deposit with a fluid component layered on top. In that case we usually address the fluid first — it responds fastest, and clearing it lets us see what is actually underneath before committing to a contouring course.
